Program Description:
Welcome to Fairleigh Dickinson University!

Please complete the medical forms required by your campus Student Health Services Office below.

Florham Campus - Medical Forms

Metropolitan Campus - Medical Forms

You may upload your completed forms to the Questionnaire for preliminary review by the Office of Student Health Services prior to submitting your original forms to complete your file.  Student Health Services will then inform you of any further requirements for you to obtain medical clearance.

Please be aware that vaccinations can cost hundreds of dollars in the United States.  We strongly recommend that you obtain medical clearance from the Office of Student Health Services prior to your arrival in New Jersey.

Medical clearance is required to enroll in classes at Fairleigh Dickinson University.
